Hospital management system with ABDM integration for a multi specialty hospital

A hospital management system covering OPD, IPD, pharmacy, lab and billing for a multi specialty hospital, with ABHA creation, ABDM health record linking and consent based sharing. Front desk, doctors and billing now work from one patient record, which replaced four disconnected tools.

Client
A 150 bed multi specialty hospital in a tier 2 city in western India with cardiology, orthopaedics and maternity departments
Region
India
Sponsor
Medical superintendent and head of finance
Duration
20 weeks to full go live across four staged releases, then an annual support plan

Engagement model

O2E: Output Oriented Engagement

Each module (registration and OPD, IPD and billing, pharmacy and lab, ABDM) was a separate accepted output, so the hospital could go live in stages without a big bang cutover.

How O2E works

The team

  • Solution strategist with hospital operations background1
  • Backend engineer2
  • Frontend engineer2
  • QA engineer (shared)1
  • Implementation lead on site1

The challenge

Where things stood

  1. Registration, OPD billing, pharmacy and the lab ran on separate software, so a patient was entered three or four times and records rarely matched.
  2. Inpatient bills were built manually at discharge from paper charge sheets, which delayed discharges and caused disputes with TPAs.
  3. The hospital needed to register on the Health Facility Registry, create ABHA numbers for patients and share records through ABDM to stay eligible for government scheme patients.
  4. Doctors had no single view of a patient's past visits, lab results and prescriptions.

The approach

How we ran it

  1. Weeks 1 to 3

    Process walk and data model

Followed patients from the front desk to discharge across three shifts. Built one patient master and encounter model that every department writes to, and agreed the charge master with finance.

  2. Weeks 4 to 9

    Registration, OPD and ABHA

Released registration with ABHA creation and verification by Aadhaar OTP or mobile, token based OPD queues, e-prescriptions and OPD billing. Completed the ABDM sandbox milestones in parallel.

  3. Weeks 10 to 15

    IPD, pharmacy and lab

Added bed management, nursing charting, running bills, TPA pre authorisation tracking, pharmacy dispensing against prescriptions and lab orders with result entry and machine interfacing for the main analysers.

  4. Weeks 16 to 20

    ABDM records and go live

Implemented health information provider and user flows so discharge summaries, prescriptions and lab reports link to the patient's ABHA and are shared only with consent. Passed the ABDM functional review and moved to production.

What we delivered

Modules and features

  • Patient registration with ABHA creation, verification and scan and share at the front desk
  • OPD queue, doctor workbench and e-prescriptions with drug and dose templates
  • IPD bed board, nursing notes and running bill visible to the patient's family on request
  • TPA and insurance pre authorisation tracking with document checklist
  • Pharmacy and lab modules linked to the same patient and encounter
  • ABDM health record linking and consent managed sharing of discharge summaries and reports
  • Role based access with audit trail for every record view

What changed

Outcomes

  • One patient record from registration to discharge, entered once at the front desk
  • Discharge bills built from charges posted during the stay
  • ABDM integration approved, so patient records link to ABHA and are shared only with consent
  • Doctors see past visits, prescriptions and lab results on one screen during consultation

Looking back

The staged go live was the right call. Running the new OPD alongside the old IPD software for six weeks was awkward, but it meant front desk staff learnt one thing at a time and the ABDM work did not hold up billing. We underestimated the lab analyser interfaces: two older machines only spoke a serial protocol and needed a small bridge on a local PC, which cost us a fortnight. Next time we would audit every device in the first week. We would also give the nursing team their own design sessions sooner, because their charting screens needed more rework than any other part.
